Bug: Create transaction action with image attachment

I have a shortcut which takes image from the share sheet as input. It performs some analysis on it and then finally passes it to create transaction shortcut. It works fine except when I have to also add the image as attachment. It works sometimes but doesn’t work other times. There are no details of error as well.
